What Is a Comprehensive Stool Analysis and What Can It Tell You About Your Gut Health?

If you have had a colonoscopy and a standard stool culture and been told everything is normal, but you are still dealing with bloating, irregular bowel movements, fatigue, or skin flare ups, you are not imagining it.

Standard testing is designed to rule out serious structural disease such as cancer, polyps, or acute bacterial infection. It is not designed to assess the day-to-day function of your digestive system.

A colonoscopy looks at the physical structure of your bowel. A standard stool culture looks for a small handful of well known pathogens. Neither test measures your microbiome balance, your digestive enzyme output, your intestinal inflammation levels, or the integrity of your gut lining. This is why so many people walk away from normal results with symptoms that are very much still present. The tests were simply never built to look at those things.

This is where a comprehensive stool analysis comes in. It is one of the most detailed tools available in functional medicine for understanding what is actually happening inside your gut, and it is a core part of how we investigate persistent digestive symptoms at Wave Functional Health on the Gold Coast.

How the test works

A comprehensive stool analysis, sometimes referred to by practitioners using the GI-MAP platform, is an advanced functional stool test that uses quantitative PCR technology to identify and measure the DNA of organisms living in your gut. Rather than relying on culturing a sample in a lab dish and waiting to see what grows, which is how standard stool cultures work, this method detects genetic material directly. This makes it far more sensitive and far more comprehensive.

The process is simple from your end. You collect a single stool sample at home using a kit, following clear instructions, and send it to the laboratory. The lab then screens the sample against a wide panel of bacteria, parasites, fungi, viruses, and other markers, and produces a detailed report that a practitioner can interpret against your symptom picture.

Because this type of functional stool testing is done through specialised laboratories rather than a standard pathology provider, it needs to be ordered and interpreted by a practitioner trained in reading these reports. This is part of why gut microbiome testing in Australia has become more accessible through functional and integrative clinics, including here on the Gold Coast.

What it measures

What a comprehensive stool analysis evaluates

A comprehensive stool analysis casts a much wider net than a standard test. Depending on the panel, it typically evaluates all of the following.

Commensal bacteria that support digestion and immunity
Opportunistic and pathogenic bacteria that can overgrow and disrupt balance
Bacterial, viral, and protozoal pathogens that cause ongoing gut irritation at low levels
Fungi and yeast including Candida species
Parasites that standard stool culture frequently misses
Zonulin — a marker of intestinal permeability
Secretory IgA — reflecting gut immune system function
Calprotectin — an intestinal inflammation marker
Pancreatic elastase — measuring digestive enzyme output

What each marker reveals and why it matters

← Scroll to see full table

Marker What it reflects Why it matters
Commensal bacteria levels Diversity and abundance of beneficial gut flora Low diversity is linked to poor digestion, weaker immunity, and mood disturbances
Opportunistic bacteria Bacteria that overgrow when the gut environment shifts Overgrowth is a common driver of bloating, gas, and irregular bowel habits
Pathogens and parasites Presence of organisms that standard tests often miss Low-grade infections can quietly drive years of unresolved symptoms
Fungal markers Yeast overgrowth such as Candida Linked to bloating, sugar cravings, brain fog, and skin issues
Zonulin Intestinal permeability Elevated levels suggest a compromised gut lining, associated with food sensitivities and systemic inflammation
Secretory IgA Gut immune activity Both very high and very low levels indicate the immune system is under strain
Calprotectin Intestinal inflammation Helps flag whether symptoms may be inflammatory in nature and require further medical investigation
Pancreatic elastase Digestive enzyme output Low levels point to poor breakdown of food, which can drive bloating and nutrient deficiencies

Who should consider a comprehensive stool analysis?

Bloating that occurs most days, especially after meals
Irregular bowel movements, whether loose, constipated, or alternating
A colonoscopy or standard stool culture that came back normal despite ongoing symptoms
Fatigue that does not improve with rest
Skin conditions such as eczema, acne, or rosacea that have not responded to topical treatment
Food sensitivities that seem to be expanding over time
A history of food poisoning, travellers diarrhoea, or a course of antibiotics before symptoms began
Symptoms of SIBO such as bloating that worsens through the day
Autoimmune conditions — gut health and immune regulation are closely linked
Brain fog, low mood, or anxiety alongside digestive symptoms

How Wave uses the results to sequence a gut repair protocol

A stool test on its own is only data. What matters is how it is interpreted and applied. At Wave Functional Health, results are read within a north-to-south functional medicine framework — meaning we look at the entire digestive process from stomach acid and enzyme output through to microbiome balance and bowel elimination, rather than treating each marker in isolation.

Sequencing matters. Introducing probiotics before clearing an underlying infection, for example, can worsen symptoms rather than improve them. The protocol is built around what your results show and in the order that makes clinical sense for your specific picture.

Your stool analysis results are combined with your symptom history and, where relevant, other functional testing such as DUTCH hormone testing or organic acids testing, to build a complete picture and a protocol that is specific to you rather than generic.

The 4R Gut Repair Sequence
1

Remove

Address any active pathogens, bacterial overgrowth, parasites, or fungal overgrowth identified on testing. This step must come first — rebuilding the microbiome without clearing underlying infections is one of the most common reasons gut protocols fail.

2

Replace

Support digestive enzyme and stomach acid function where pancreatic elastase is low or hypochlorhydria is present. Poor enzyme output means food is incompletely digested before it reaches the lower gut, driving fermentation and bloating.

3

Repair

Repair the gut lining where zonulin is elevated or secretory IgA indicates mucosal immune strain. Targeted nutrients support intestinal barrier integrity and reduce the systemic immune activation that drives food sensitivities and inflammation.

4

Reinoculate

Introduce targeted probiotic strains and prebiotic support to rebuild a diverse, resilient microbiome. Strain selection is guided by what the testing shows rather than a generic probiotic protocol.

Frequently asked questions

No. Functional stool testing such as GI-MAP is not a Medicare rebated test in Australia. It is ordered privately through a functional medicine practitioner and the cost is paid out of pocket. Dr Matt will discuss the cost and what the test is likely to reveal before ordering it so you can make an informed decision.

A standard GP stool culture typically screens for a small number of common pathogens using culture methods — where a sample is grown in a lab dish and observed for a limited set of organisms. A comprehensive stool analysis uses DNA-based technology to screen for a much wider range of bacteria, parasites, fungi, and functional markers such as inflammation, digestive enzyme output, and intestinal permeability.

The two tests are simply not looking for the same things. A normal GP stool result and a normal comprehensive stool analysis are not equivalent statements.

No referral is required. The test is ordered directly through Wave Functional Health as part of your initial consultation and investigation plan. You do not need to see your GP first or get a referral from a specialist.

Results typically take two to three weeks from the time the sample is received by the laboratory. Dr Matt will book a Report of Findings appointment with you once results are back to walk through everything in detail and present your protocol.

A comprehensive stool analysis can support a SIBO picture by identifying dysbiosis patterns and small intestinal overgrowth markers, but it is not a direct SIBO diagnostic test. SIBO is formally diagnosed through breath testing, which measures hydrogen and methane gas produced by bacteria fermenting carbohydrates in the small intestine.

At Wave we often use both stool analysis and breath testing together to get the most complete picture where SIBO is suspected alongside broader gut dysfunction.

Not at all. It is a simple at-home collection kit requiring a single stool sample. There is no fasting, no bowel preparation, no procedure, and no clinic visit required for the collection itself. The kit comes with clear instructions and is posted directly to the laboratory once collected.

If you have been told your gut is fine but you know something is still not right, a comprehensive stool analysis may be the test that finally connects the dots. At Wave Functional Health in Robina on the Gold Coast, we combine advanced functional testing with a structured, sequenced approach to gut repair so you are not left guessing at supplements or restrictive diets without knowing why.
